The NSM SGS Mental Health Community team is seeking a Community Support Worker (CSW) to work collaboratively within an interdisciplinary team to support older adults and their care partners (formal/informal) living with cognitive impairment and responsive behaviours in community-based settings across the NSM region.

The successful candidate will provide assessment and consultative services for clients experiencing behavioural challenges related to mental health and/or cognitive impairment.

The successful candidate will bring expertise in geriatric mental health (including responsive behaviours) and specialized geriatric services to improve patient outcomes, build capacity and foster system change.
